Registration
Information
At
Renaissance
Hospital Terrell,
admissions are
based on your
physician’s order
and are scheduled
in advance, except
in emergency
situations. Since
almost all types
of insurance
require
pre-authorization
or certification
prior to the
hospital
admission, we may
call you at your
home in advance to
obtain
information. Upon
arrival at the
hospital, the
Registration Clerk
will process
information for
your hospital
record and will
help you complete
insurance and
"Consent for
Treatment" forms.
Please be prepared
to show some form
of identification
and sign the
necessary
insurance forms.
Depending on the
individual, this
process could take
up to 15 minutes
of your time.
Parents must
accompany minor
patients. Children
under the age of
18 cannot be
admitted without
the consent of a
parent or guardian
at the time of
admission.
Guardians must
bring proof of
guardianship to
the hospital. If a
young child is
admitted, an adult
must be with that
child at all
times.
All patients will
be admitted, rooms
assigned and
treatment
administered
without regard to
race, color,
religion or
national origin.
Private and
semi-private rooms
are based on
availability.
Items to bring
with you to the
hospital:

medications
you are
presently
taking (do
not bring the
actual
medications
with you
unless
instructed to
do so by your
physician).

Personal
nightwear,
including
robe and
slippers.

Essential
toiletries
(i.e.,
toothbrush,
toothpaste,
comb, brush
and
containers
for dentures,
contact
lenses,
eyeglasses or
hearing
aids).
Items not to
bring with you to
the hospital:
